Giving your injection

“Sub-Q” means under the skin. It’s quick, and after the first time most people say it’s easier than they expected.

Giving a sub-Q injection
What is sub-Q?

“Sub-Q” is short for subcutaneous, which means “under the skin.” Alex has prescribed a medication that you give yourself by injection (a shot) under your skin.

You can give it yourself, or a caregiver can give it for you. Alex will walk you through it — then follow the steps below each time.

Before you start

Check which applies to you:

Rotating your spot

Why “a different spot each time” matters — and how to keep track.

1234belly button

Injecting in exactly the same place every week can irritate the skin and the tissue underneath. So each week, you move to a new spot.

Your three areas

  • Belly — around your belly button
  • Thighs — the front of either thigh
  • Back of the arms — either arm

On your belly

Picture a clock around your belly button. Pick a direction — clockwise or counter-clockwise — and move one spot round each week.

Example 1 · around the belly

  • Week 1: above your belly button
  • Week 2: to the right
  • Week 3: below
  • Week 4: to the left
  • Week 5: back to the top

Example 2 · mix it up

  • Week 1: belly
  • Week 2: left thigh
  • Week 3: right thigh
  • Week 4: back of left arm
  • Week 5: back of right arm

Side effects & when to reach out

Most side effects are mild and ease as your body adjusts. Here’s what’s normal — and when to call us.

    Common

    • Nausea, reflux or heartburn, less appetite
    • Constipation or diarrhea
    • Bloating, mild stomach discomfort, tiredness
    • Headache (often from not eating or drinking enough)

    What helps

    • Smaller meals, eat slowly, stop when satisfied
    • Protein first; skip greasy or fried food early on
    • Sip water all day; add electrolytes if needed
    • Add fiber slowly
Message the team the same day if you have:
    • Vomiting that won’t stop, or you can’t keep fluids down
    • Severe constipation or severe stomach pain
    • Worsening reflux or signs of dehydration

    Your daily targets

The simple numbers that keep your progress steady.

    Why it matters

Your medication turns your appetite down — so what you do eat matters more than ever. Getting enough protein and water every day protects your muscle, helps keep side effects calmer, and gives your body what it needs to keep losing weight steadily.

These are the same goals Alex asks about at every follow-up. Tracking them turns your 15-minute visit into a real conversation about what’s working.

    Your targets

    • Protein at every meal — Alex gives you your personal daily target
    • Roughly 40% protein, 40% good fats, 20% carbs — carbs mostly from fruit and vegetables
    • At least 100 oz of water a day
    • Track it in MyFitnessPal or any app you like
    • Limit processed and fast food as much as you can
    Healthy habits · Nutrition
    Protein first!

    Protein made easy

Your most important nutrient on a GLP-1.

    Why it matters

When you lose weight, you can lose muscle along with fat — especially if you’re not eating enough protein. Protein is what protects that muscle, and it keeps you fuller for longer.

With a smaller appetite, the trick is to eat your protein first at every meal, then everything else. Here’s what that looks like on your plate.

    Tips & easy wins

    • Eat your protein first at every meal
    • Budget-friendly: eggs, canned tuna or chicken, Greek yogurt, cottage cheese, beans, lentils, rotisserie chicken, frozen burgers, protein shakes
    • Feeling queasy? Try gentle protein: yogurt, cottage cheese, a protein shake, turkey slices or scrambled eggs
    • Clean protein powders Alex suggests: Paleo Valley, Just Ingredients, Equip, Orgain (not a full list)

What 30 g of protein looks like

4 ozchicken, turkey, lean beef or pork loin
    5 ozsalmon or shrimp
    4large eggs
    1 cupegg whites
    1¼ cupscottage cheese
    1½ cupsplain Greek yogurt
    1½ cupstofu, lentils or edamame
    1¾ cupsblack beans

    Nuts count too: ½ cup peanuts ~17 g · almonds ~14 g · pistachios ~13 g · sunflower seeds ~12 g · cashews ~10 g · walnuts ~9 g · 2 tbsp peanut butter 7–8 g

    Healthy habits · Nutrition
    Fiber is your friend.

    Carbs & fiber

Choose carbs that work for you, not against you.

    Why it matters

Carbs aren’t the enemy — the right ones bring fiber, which keeps you full and keeps things moving. That matters, because constipation is one of the most common side effects on these medications.

Build your meals around protein and produce first, then add carbs based on how hungry and active you are. Go slowly with fiber — too much too fast can cause gas and bloating.

    Tips

    • Choose carbs that bring fiber: oats, brown rice, potatoes, beans, fruit, whole-grain bread, tortillas, frozen veggies
    • Skip “liquid calories” — soda, sweet tea, juice
    • Work up to 25–35 g of fiber a day over time
    • Easy fiber add-ons: beans, lentils, oats, chia seeds, berries, apples, frozen vegetables, salads
    • Psyllium can help with regularity — start small and ask Alex if you’re unsure
    Healthy habits · Nutrition
    Drink up!

    Water & electrolytes

The easiest habit with the biggest payoff.

    Why it matters

Not drinking enough can make nausea and headaches worse — two things nobody wants. Staying hydrated is one of the simplest ways to feel better while your body adjusts.

Aim for water all through the day rather than all at once, and top up electrolytes on days you’re not eating much.

    Tips

    • Aim for at least 100 oz of water a day
    • Keep a bottle with you and sip all day
    • Add electrolytes if you’re not eating much or you feel light-headed
    • Headache? Check your water first

    Eating when you’re not hungry

Your appetite will change — here’s how to fuel well anyway.

    Why it matters

Most people notice less hunger within the first one to two weeks. That’s the medication working — but it can make it easy to under-eat, especially protein.

Smaller, slower meals are easier on your stomach and help you get the good stuff in before you feel full.

    Tips

    • Eat smaller meals and slow down — stop when you feel satisfied
    • Eat your protein first
    • Avoid greasy or fried food, especially early on
    • A protein shake counts when a meal feels like too much

    Smart shopping

A better week starts in the grocery store.

    Why it matters

What’s in your kitchen is what you’ll eat. The outside edge of the store — produce, meat, dairy — is where the whole foods are. The middle aisles are mostly processed.

Beans and nuts are the exceptions worth grabbing from the middle. Here’s a starter list from Alex’s food guide.

    Your starter grocery list

    Vegetables

Leafy greens, broccoli, cauliflower, Brussels sprouts, bell peppers, cucumber, zucchini, tomatoes, mushrooms, asparagus, green beans

    Supplements

Optional extras — always check with Alex first.

    Why it matters

Supplements aren’t required, but a few can help fill the gaps when your appetite is small or your energy is low.

Talk to Alex before you start anything new, so she can make sure it fits your plan.

    What to ask Alex about

    • Protein powder or shakes — an easy way to hit protein when appetite is low
    • Vitamin D — worth checking your levels; supplement if you’re low
    • Magnesium glycinate in the evening may help some people relax and sleep
    • Creatine monohydrate supports strength training and muscle
    • Psyllium fiber can help with regularity — start low
    • Low energy? Ask about checking iron, B12, vitamin D and thyroid
    • Avoid “fat burner” stimulant supplements unless Alex has reviewed them with you

    Why strength matters

The most important habit on a GLP-1.

    Why it matters

When you lose weight, you can lose muscle and bone density too — especially without enough protein and strength training. Strength training is how you protect them.

It also supports a healthier metabolism, so the weight you lose is more likely to stay off. Protein plus strength training is the combination Alex asks every patient to focus on.

    Tips

    • Pair every strength session with protein that day
    • Start with bodyweight moves if you’re new

Once you’re comfortable, this is the plan Alex recommends.

    Your week

    • Move for at least 30 minutes every day
    • Strength or resistance training 3–4 times a week, 30–45 minutes
    • Cardio on a different day from strength — or after your strength session
    • On a treadmill, add incline a little at a time
    Healthy habits · Movement
    Every journey starts small.

    Starting from zero

    Meet yourself where you are.

    Why it matters

You don’t need a gym or a plan that exhausts you. Small, realistic steps are how every strong habit starts.

Start small, keep it regular and build up week by week.

    How to begin

    • Start with a 5–10 minute walk after meals, 3–4 days a week
    • Build towards 150 minutes a week of brisk walking
    • Add strength training 2 days a week when you can, then build up
    • No gym needed: bodyweight squats, wall push-ups, chair sits, resistance bands
    Healthy habits · Movement
    Just 15 minutes.

    Walking after meals

A small habit with a big payoff.

    Why it matters

A short walk after you eat helps your body handle blood sugar — and it supports weight loss too.

You don’t need a long walk. Fifteen minutes within an hour of eating is enough to make a difference.

    Tips

    • Walk for about 15 minutes within an hour of eating
    • Can’t do long walks? Short ones after meals still count
    • Make it social — walk with a friend, partner or your dog

    It all adds up.

    Everyday movement

    It all adds up.

    Why it matters

The movement you do outside of workouts matters more than most people think.

Every extra step through the day adds up to real results.

    Easy wins

    • Take the stairs
    • Park further away
    • Take short walking breaks during the day
    • Household chores count too
    Healthy habits · Movement
    Sweet dreams.

    Sleep & recovery

Rest is part of the plan.

    Why it matters

Poor sleep increases the hormones that make you hungry, drives cravings and can slow your progress.

Consistency matters more than perfection — a regular routine makes the biggest difference.

    Tips

    • Aim for 7–9 hours
    • Keep the same bedtime and wake time
    • Dim the lights an hour before bed; keep your room cool and dark
    • Limit screens and heavy meals late at night
    • Cut caffeine after early afternoon if sleep is a struggle
